Our Integrated Care Services provide case management services including comprehensive care management, care coordination, health promotion, comprehensive transitional care, Individual and family support services, and referrals to community and social support services.

Patients who need help and guidance finding services or resources are invited to meet with Jayda, our Care Manager at Heart of Kansas.

Jayda Lauber
Jayda Lauber was born and raised in Great Bend, KS. She attended Barton County Community College where she received her Associated in Science. She attended Kansas State University where she obtained her Bachelor of Social Work. While maintaining a full-time student status, Jayda also worked in the community in fields that would build her social work skills. Jayda has plans of returning to school to further her education in addiction counseling.
